The City of Ocoee requires a Vendor Application and the IRS W-9 Form in order to be added to the City's vendor list. Your account cannot be setup if both forms are not submitted together. Please submit completed forms by fax to (407) 905-3194 or by email to MHill@ci.ocoee.fl.us
